Applicant           1. Must attend Seminar on Water Services Connection held every Wednesday, 3:00 p.m. and every Saturday, 9:00 a.m. at COWD's Social hall, 4 th floor, main office. 2. Fills out Pre-application Form and submits to customer service section for data encoding. 3. The applicant files his/her application and submits the requirements to COWD'S Customer Service Section located at the Corrales Avenue . The applicant must submit any of the following requirements applicable to his/her application:       APPILCANT OWNS THE LOT   1. Lot Title or Tax Declaration   2. Deed of Sale       APPLICANT RENTS THE UNIT   1. Lease Contract       APPLICANT DWELLS IN PUBLIC LAND   1. DENR Certification   2. Certification from Barangay Captain   3. Affidavit of Waiver       APPLICANT RESIDES IN SUBDIVISION   1. Certification from Subdivision Developer   2. Assumption of Mortgage       CERTIFICATE OF ATTENDANCE         INSPECTION AND VERFIFICATION - COWD's inspectors check out the place of the applicant to determine its distance from the District's mainline, the materials required, the water pressure and the estimated amount to be paid by the applicant.
